About your visit
To assist in your comprehensive oral evaluation and treatment as a new patient, please read our New Patient Welcome Letter and download, print, and complete the following forms prior to your appointment:
You are strongly encouraged to complete the required medical and dental history forms via our secure patient portal. Please bring pertinent dental records, X-rays, etc., to your appointment, especially if you seek a second opinion.
You will receive an appointment-reminder phone call or email 2-3 days prior to your scheduled appointment.
Please plan to arrive about 15 minutes before your appointment.

Before your appointment
Complete your Medical History in MyChart
Patients must complete their Medical History questionnaire in MyChart prior to their visit. Please review and update your medications, allergies, health conditions, and any recent changes before your appointment.
Completing this information ahead of time helps prevent delays and allows your provider to complete your appointment as scheduled.

MyChart provides secure access to your health information and may be used to:
- Complete your Medical History.
- Update your demographics and insurance information.
- View upcoming appointments.
- Access visit summaries and treatment plans.
- Send non-urgent messages.

What to bring
Please bring the following to your appointment:
- A valid photo ID.
- Your current insurance card.
- A current medication list if your medications have not already been updated in the patient portal.
Arriving for your appointment
